 這病…外觀變化緩慢 診斷晚5-7年


73歲王先生是位洗腎病人，多年來血壓偏低，洗腎時收縮壓常常低於60mmHg導致無法順利完成透析，來醫院求助。王先生10年前曾在醫學中心診斷肢端肥大症，已順利手術切除腫瘤，但術後漸漸血壓偏低、精神不振、視力模糊，加上洗腎不順，十年來生活品質並不理想。

收治患者的台南市立安南醫院陳擇穎醫師發現，王男雖順利接受手術切除和放射治療，腦下垂體腫瘤也已消失，卻以為已經痊癒而缺乏長期追蹤，抽血發現生長激素指數仍非常高。反之，其他正常賀爾蒙卻因治療而分泌不足，雄性賀爾蒙和腎上腺激素嚴重低下，因此導致各種不適症狀。

醫師協助王男申請針對生長激素抑制的特殊用藥（體抑素衍生物），並補充缺乏的多種賀爾蒙後，各項內分泌指數都回到正常值；王男感到身體大幅改善，連洗腎時血壓也逐漸回穩，視力模糊也顯著進步，令洗腎醫師嘖嘖稱奇。

新陳代謝科陳擇穎醫師指出，肢端肥大症為罕見疾病，是腦下垂體分泌生長激素過多造成的疾病，每百萬人一年才會診斷3-4人。如果青春期發生會變巨人症，而成年後則是造成全身軟組織、骨骼、內臟增生肥大，導致外貌改變（臉變寬變粗、手腳變大），以及糖尿病、高血壓、心臟病等風險增加，若不治療平均比正常人少活10年。台灣目前大約只有1500位患者，由於疾病罕見，症狀變化緩慢，民眾甚至醫師也常忽視其症狀，導致診斷時往往已經發病多年。

手術切除腦垂體腺瘤為治療首選，若有殘餘腫瘤考慮放射治療。然而，一些病患即使已經完成療程，仍可能生長激素分泌過多，並且合併術後導致的全腦下垂體分泌不足，此時就需要內科藥物治療介入，所以術後的定期追蹤是必須的。

陳擇穎醫師表示，肢端肥大症除了診斷不易外，病患可能在切除其垂體腫瘤後誤以為就此痊癒，而忽略後續的追蹤及藥物治療，建議病患若有相關問題可至內分泌專科做進一步檢查，才不會延誤病情。
